Consent Form

Here is a standard Consent or Emergency Contact Form that may be filled out by parents.
It's good practice for home child care providers to carry this type of form with them at all times.
Better safe than sorry!

Clients: If your contact info is out of date (change of address, etc.), just fill out this form and hit the Submit button.
It will automatically generate a new one for us, that I will print out to replace the old one.
*Parent's or Guardian's Name(s) *Name of Child/Children in care *Date of Birth(s) (Month, Day, Year) Home Address, Telephone number(s), email (optional)
Mother's Place of Work, Address, Contact Info
Father's Place of Work, Address, Contact info
Child's Health Card Number and version code (optional, but very helpful in case of emergency Medical Info: Doctor's Name and Contact Info. Please use this section to state if child has any allergy or medical information child care provider should know about:
In Case of Emergency: In the event that I am unable to pick-up my child from the caregiver's home, my child(ren) may be released to these people. Name(s), Address, Contact Info
